At Center for Discovery we understand the importance of professional collaboration to effectively facilitate residential care for adolescents and their families. That's why our goal is to work diligently with referring professionals to ensure successful continuum of care in teen treatment. We will setup a mutually agreed upon Communication Plan during your client's treatment with us and provide an extensive Discharge Plan upon the release of your client back to your care.
